Pre-Participation Sports Forms

(for students in grades 7-12 who wish to participate in interscholastic athletics or extracurricular activities)

HCS Middle and High School students planning on participating in interscholastic athletics during the 2010-11 school year must have a pre-participation physical on file in their high school's athletic office/athletic training room before they can participate in practices and games. Physicals for the 2010-11 school year are valid from April 1, 2010 - June 30, 2011. Students and parents are encouraged to obtain a physical as soon as possible in anticipation of the upcoming sports seasons.  orms are available through your school's athletic training room or may be downloaded here.

If you should have any questions or concerns about this process, please do not hesitate to contact the Athletic Trainer at your child’s high school.

NOTE: Physical forms MUST be signed by a licensed medical doctor in South Carolina or a Certified Physician’s Assistant or Family Nurse Practitioner practicing under the supervision of a licensed South Carolina MD or DO.  Chiropractor signatures are NOT valid!!

Turn ALL forms to the sports medicine department at your child’s high school before their first practice.
